I. Team & Enrollment
Since 61% of our neighbors speak Spanish, we added a Bilingual Peer Support Intern. This changed everything. We grew from 42 to 100 members who we now help every day.
Having someone who speaks the language built trust with families.
We assist our 100 members as they move through the SDoH system.
Families feel comfortable sharing health needs because they are understood.
II. Health & Wellness Wins
Milestone: 45 clients finished the full health service flow.
Maternal Health
We helped a pregnant mom with high blood sugar stabilize her pregnancy through diet.
Chronic Care
Helping members manage Diabetes and Asthma in their own language.
PCP Linkage
Connected families to doctors and reduced their fear of medical visits.
Housing
Referred 3 unhoused people to programs; all three are still successfully housed.

Conclusion
By adding bilingual staff and using PearSuite, we've turned our outreach into a real care program. We assisted 100 members this quarter and finished 45 full health cases.
