Community Health Worker Care Table
Our friendly Community Health Workers help you with everyday life and health needs. Get support with food, housing help, medical appointments, and ride assistance. This service is for people who have these Medi Cal or Medicaid health plans: Health Net, Molina, Anthem, and L.A. Care. Please bring medical ID card, and ID or benefit verification letter.
